Once your child has been offered a spot in the program, enrollment is official when the following forms are completed online/submitted and the tuition deposit is paid.

Certificate of Immunization

• Certificate of Medical Exemption to vaccines signed by a licensed health care provider must be submitted once, unless your child’s information changes. Find the certificate HERE.

• Certificate of Non-medical Exemption must be submitted annually. There are two ways to file a non-medical exemption.

  • File the Certificate of Nonmedical Exemption WITH a signature from an immunizing provider, OR

  • File the Certificate of Nonmedical Exemption received upon the completion of CDPHE’s online education module.
